Band 5 - Registered Mental Health Nurse - Portsmouth

About The Role:

The role of the RMN in Portsmouth Hospital University NHS Trust is to work in partnership with the multidisciplinary team to provide enhanced support to patients with complex mental health needs in the acute care environment. The RMN should provide therapeutic intervention and engagement, whilst also assisting patients to undertake their activities of daily living. 

Responsibilities:

  • Carry out nursing care to the highest possible standard according to Trust Policies and Practice and Clinical Guidelines. This requires a thorough understanding of policies and guidance related to for example: infection control, hygiene and individualised patient needs.
  • Complete and update care plans, risk assessments and risk management plans for the patients’ direct care and support on the ward. Liaising with MHLT, relevant professionals, family and friends for relevant information.
  • To assist patients in all areas of their daily personal care and hygiene needs according to their care plans.
  • Contribute to a full range of routine care tasks such as supporting self-care and completing routine observations, including physical care (washing, dressing) and observations when required.
  • To assist in the delivery of therapeutic intervention for our patients under the guidance of the medical and mental health liaison teams (MHLT). Reporting any significant changes in mental state to MHLT.
  • To engage in conversations to help maintain patients' sense of identity, upholding dignity at all times. Role modelling compassionate mental health care to staff on the wards.
  • To support patients to engage in activities of daily life that promotes a sense of purpose, belonging, and achievement.
  • Supporting patients detained under the Mental Health Act to understand their rights. If they wish to appeal helping them to complete necessary paperwork and accompanying them to the tribunal.
  • Liaising with patients’ family and friends to provide updates on progress and discharge plans.
  • Contribute to the nursing team’s knowledge and awareness of the changing needs of the patient.
  • Utilise and promote de-escalation skills to prevent incidents of self-harm or harm to others.
  • Risk assess activities outside of the ward, considering any legal frameworks that may be in place.
  • Escalate the need for PRN medication in a timely manner to the medical/ward team.

Please note that the RMN pool shifts are allocated based on need and therefore flexibility of where the RMN will be working is required.

RMN Pool allocation and redeployment:

07:30 Early shift - RMNS to report to Hospital at night team D level IDS suite

12:30 Late shift - To report to the operation centre on C level

19:30 Night shift - RMNS to report to Hospital at night team D level IDS suite or bleep 0119

Requirements:

  • NMC Registration
  • At least 6 months working within mental healthcare
  • Strong clinical knowledge, good initiative, and a proactive approach to work
  • Excellent communication skills, both written and verbal
  • Honesty and integrity, and a caring patient-centred approach
